  Shijiazhuang Yongtong Chemical Co., Ltd.'s relocation and reconstruction project with an annual output of 3,000 tons of aminophenol and 150 tons of sodium p-aminosalicylate (the first phase with an annual output of 1,500 tons of aminophenol)
2. Introduction to the safety assessment project:
  Shijiazhuang Yongtong Chemical Co., Ltd. was originally located at No. 261 Shengli South Street, Shijiazhuang City. In May 2009, according to the urban planning requirements of Shijiazhuang City, the company was relocated to the west of Lirenzhuang Village, Huaiyang Town, Yuanshi County (Chengnan Industrial Park, Yuanshi County). On May 6, 2009, the company obtained the record certificate of Yuanshi County Development and Reform Bureau on the annual output of 3,000 tons of m-aminophenol, 150 tons of sodium p-aminosalicylate, and 500 tons of m-hydroxyl N, N-diethylaniline project (certificate number: Yuan Fa Changbei Zi [2009] No. 13).
  In May 2010, the company has commissioned Hebei CITIC Safety Technology Service Co., Ltd. to conduct a safety evaluation and passed the review of the experts of the Yuanshi County Safety Supervision Bureau (Yuan Safety Supervision Hazardous Chemical Project Wenzi [2010] No. 14). The scope of the safety evaluation is "the relocation and transformation project with an annual output of 3,000 tons of m-aminophenol, 150 tons of sodium p-aminosalicylate, and 500 tons of m-hydroxyl N, N-diethylaniline".
  In November 2010, the company commissioned Sinochem Chemical Science and Technology Research Institute to complete and pass the review of the special design of safety facilities. The design scope of safety facilities is "the relocation and transformation project with an annual output of 3,000 tons of m-aminophenol, 150 tons of sodium p-aminosalicylate, and 500 tons of m-hydroxyl N, N-diethylaniline".
During the construction process, due to market reasons, the company only built an annual output of 1,500 tons of inter-aminophenol production plant and public works and auxiliary facilities. On June 20, 2012, with the approval of Yuanshi County Development and Reform Bureau (Yuan Fa Gai [2012] No. 29), no longer builds an annual output of 500 tons of inter-hydroxyl N, N-diethylaniline project. On February 26, 2015, the company submitted a request for the phased construction of the project. The first phase of the construction of the 1,500 tons of inter-aminophenol project and the second phase of the construction of the 1,500 tons of inter-aminophenol and 150 tons of p-aminosalicylate sodium project obtained the consent of Yuanshi County Development and Reform Bureau.
   According to Article 14 of the "Hebei Province Detailed Rules for the Safety Supervision and Administration of Hazardous Chemicals Construction Projects" (Ji'an Supervision No. 3 [2012] 146): "Construction projects that have passed the safety condition review shall be re-evaluated by the construction unit approved by the relevant departments." The company needs to re-evaluate the safety conditions.
   The scope of this safety condition evaluation is the first phase of a project with an annual output of 1,500 tons of aminophenol.
Construction project investment: the first-phase construction investment is 18 million yuan, and the investment in safety facilities is estimated to be 1.008 million yuan, accounting for 5.6% of the first-phase project investment;
   The main content of the first phase of construction:
The first phase of the main construction of aminophenol workshop, sodium aminobenzenesulfonate workshop, boiler room, heat transfer oil room, storage tank area, class B warehouse, raw material warehouse, class E warehouse, sodium sulfite warehouse, control room, air pressure room, circulating fire protection pool, sewage treatment station, accident pool, office, comprehensive room, workshop office, etc., purchase production equipment and facilities and supporting public works and auxiliary facilities.

4. List of personnel, time, and main tasks to carry out safety evaluation work on site, and evaluation report time:
  In April 2016, Shijiazhuang Yongtong Chemical Co., Ltd. approached Hebei Huafei Technology Consulting Co., Ltd. to discuss the safety condition evaluation of its proposed relocation and transformation project with an annual output of 3,000 tons of m-aminophenol and 150 tons of sodium p-aminosalicylate (the first phase with an annual output of 1,500 tons of m-aminophenol). Huafei Company appointed relevant personnel to visit the site of Yongtong Chemical Company to conduct on-site inspections, understand the main equipment, production scale, general layout, public works supply and surrounding relationships of the project, and collect relevant information of the project, and make relevant risk analysis of the project. After many conferrals, the relevant personnel of the two companies formed a preliminary intention and initially clarified the object and scope of the evaluation.
  During the evaluation process, the members of the evaluation team carefully analyzed and sorted out the relevant materials provided by the enterprise and collected on site, identified the dangerous and harmful factors existing in the project, conducted a qualitative and quantitative evaluation of the inherent danger degree of the project according to the divided evaluation units, and analyzed the risk degree of the project, put forward targeted safety countermeasures and suggestions, and exchanged views with the construction unit. On this basis, a safety evaluation conclusion was made. After solid work, the safety condition evaluation report of the project was prepared and submitted for review in May 2016. In June 2016, Shijiazhuang Safety Production Supervision and Administration Bureau organized experts to review the report and put forward revised opinions. In July 2016, the evaluation team made serious revisions to the report in accordance with expert opinions and delivered it to the construction unit, which was submitted to the Municipal Safety Supervision Bureau for administrative approval.
