2. Introduction to the safety assessment project:
Huanghua Jinhua Additive Co., Ltd. was established in 1988. It is a joint-stock enterprise jointly established by technical experts from Tianjin and Huanghua. The production base is located in Chengxi Economic Park, Huanghua City, Hebei Province, that is, southeast of Nanwangman Village, Tengzhuangzi Township, Huanghua City. The company mainly produces selenium dioxide, selenite, sodium selenite, zinc selenite, calcium iodate, potassium iodate and copper sulfate, with a total capacity of 369 tons.
Huanghua Jinhua Additive Co., Ltd. obtained the safety production license issued by the Hebei Provincial Safety Production Supervision and Administration Bureau in 2007, and replaced it with a new safety production license in 2009 and 2011. The company carried out the design safety diagnosis transformation of the in-service device in May 2014 according to the relevant requirements, started practice running in June 2014, and completed the construction of the safety design diagnosis change content at the end of October 2014.
Huanghua Jinhua Additive Co., Ltd. has a sound organizational structure, with financial department, production department, safety department, supply and marketing department, inspection department, office and other functional areas of business. The company currently has 81 employees. Huanghua Jinhua Additive Co., Ltd. attaches great importance to safety production, has a large amount of safety capital investment every year, and actively promotes the safety standardization construction of hazardous chemical enterprises, and has achieved the requirements of the third-level standard. The company has established and improved the "three systems", formulated emergency rescue plans and conducted drills, and set up a special safety management organization - the safety department, with 3 full-time safety management personnel. The main person in charge of the company and the safety management personnel have passed the safety qualification training organized by the Safety Production Supervision and Administration Bureau and received regular safety further education. They have the qualifications for safety production management. Special equipment operators and special operations personnel have undergone professional training or review and obtained qualified certificates. The staff of each position have passed the Security Training organized by the unit and passed the assessment, and have the qualifications for safety production.
According to the safety evaluation contract signed by Hebei Huafei Technology Consulting Co., Ltd. and Huanghua Jinhua Additive Co., Ltd., the safety evaluation object of this evaluation is the completion of the transformation of the safety facilities (design safety diagnosis and change) of Huanghua Jinhua Additive Co., Ltd. and the current situation of safety production. The specific scope includes the safety management of Tianbao Company, site selection, general layout, construction (structure) buildings, production equipment, storage facilities, public works and auxiliary facilities, design safety diagnosis and change rectification completion.

4. List of personnel, time, and main tasks to carry out safety evaluation work on site, and evaluation report time:
In June 2014, Huanghua Jinhua Additive Co., Ltd. approached Hebei Huafei Technology Consulting Co., Ltd. to discuss the safety status evaluation of the company. Huafei Company appointed relevant personnel to visit the site of Huanghua Jinhua Additive Co., Ltd. in July 2014 to conduct on-site inspections to understand the safety design diagnosis, main equipment, production scale, general layout, public works supply and surrounding relationships of the project, and collected relevant information of the project. Relevant personnel of the two companies have conferred many times and formed a preliminary intention to clarify the object and scope of the evaluation. During the evaluation process, the members of the evaluation team carefully analyzed and sorted out the relevant materials provided by the enterprise and collected on site, went to the unit where the project was located to survey the site, invited relevant personnel to introduce the relevant situation of the project, and conducted a detailed investigation of the surrounding environment and plant site. The dangerous and harmful factors existing in the project were identified, and reasonable and feasible safety countermeasures were proposed for the hidden dangers existing in the project. The evaluation report was completed in September 2014, and all the work of the project was completed in December 2014.
